Global Youth Camp

By Checking & Tying your name below you agree: As a camper I agree to abide by all the rules set forth by the Tabernacle Youth Camp Leadership Team. I further understand that failure on my part to comply with rules and directions given by the event organizers and youth leaders that are serving as counselors and chaperones of the event will merit my immediate dismissal from the Tabernacle Global Youth Camp at my parent/guardian’s expense.

By Checking and Typing your name below you agree: As a parent/guardian I agree to allow my child to attend the 2019 Tabernacle Youth Camp at Global. I understand that the event organizers, the Tabernacle, and Global Youth Camp will not be held liable for injuries incurred as a result of my child’s attendance at this event. Should injury or illness arise, other than minor injury, I give my permission for medical care to be provided. I shall be fully responsible for payment of such cost. I also give my permission for any photographs taken of my child to be used for possible promotion of camp. I also give my permission for my child to be transported by the Adult Leaders while on this trip. I acknowledge that if my child should fail to comply with the rules set forth by Tabernacle's Leadership and/or Global Youth Camp, my child will be immediately dismissed from camp at my own expense.

GLOBAL YOUTH CAMP PARTICIPANTS RELEASE AND WAIVER OF LIABILITY FORM I am the parent of the above named camper who will be participating in Global Youth Camp on June 10-14, 2019. I recognize that there are risks involved in participating in this trip and hereby assume all risk of injury, harm, damage, or death in connection with my participation in it. I understand and agree that neither Global Youth Camp and/or Tabernacle Baptist Church nor its trustees, officers, directors, employees, agents, or representatives may be held liable in any way for any injury, harm, damage, or death that may occur to my child as a result of their participation in the Youth Camp. To the fullest extent permitted by law, I agree to save and hold harmless Global Youth Camp and/or Tabernacle Baptist Church, its trustees, officers, directors, employees, agents, and representatives from any claim by myself, my estates, heirs, successors, assigns, or other persons arising out of my child's participation in the Youth Camp. I authorize Tabernacle Baptist Church through is trustees, officers, directors, employees, agents or representatives to render or obtain such emergency medical care or treatment for me as may be necessary should any harm, injury, or accident occur to my child while participating in the mission trip. I understand and acknowledge that Global Youth Camp and/or Tabernacle Baptist Church does not provide health or medical insurance with the mission trip and I agree that I will be financially responsible for any bills incurred as a result of medical treatment, including emergency medical treatment, and/or transportation to a medical facility, in connections with my child's participation on the mission trip.
